Our free 15-minute phone consultation gives you the opportunity to ask questions and take the first step toward the care your child deserves.

To get started, please complete the consultation form by clicking the button below. This form will provide us with general information about your child and their therapeutic needs. Our clinical team will review your form to learn more about your family’s needs and to determine if we are a match for services.

Once our consultation form is completed, you will be automatically redirected to our calendar to schedule a 15 minute phone call with our scheduling team to discuss billing, scheduling, and any additional questions.

We currently accept Medicaid, Anthem/BCBS, UnitedHealthcare, Cigna, Zero Health, and most CHP+ plans. We are not in network with Kaiser, Tricare, or AETNA. If we do not accept your insurance, we will provide you with documentation needed to get a partial reimbursement as an out-of-network provider.
